Description

‘Activate Your Heart®’ is an online cardiac rehabilitation programme that has been designed by cardiac rehabilitation specialists and patients at the University Hospitals of Leicester NHS Trust. The aim of the programme is to help those people who have had a recent cardiac event or have an existing cardiac problem, manage their condition more effectively.

It is recognised that cardiac rehabilitation improves patient life and has been recommended in international guidelines. It is also recognised that it would generate cost savings for healthcare providers: in the UK, current cost of cardiac readmission is £3637/patient, compared to the cost of delivering a good CRP of £477/patient.
